source from ***.******.*** （*）必须是生产企业或生产企授权的代理商或省级以上级别的区域代理商；
source from ***.******.*** （*）投标医疗器械产品必须具有《医疗器械生产或经营企业许可证》（备案证）、《医疗器械注册证》、《医疗器械产品注册登记表》；
source from ***.******.*** （*）本项目不接受联合体形式。

source from ***.******.*** （*）标书：应包括企业法人营业执照、投标项目相关的许可证及资质证明，企业法人授权书、授权人及被授权人身份证的复印件（以上证件须在有效期内，提供的复印件须加盖企业公章）、相关产品资质、产品业绩（需附业绩列表和发票复印件）及生产厂家资质和授权等，如产品在甘肃省阳光采购平台，需列明阳采编码。
